With chronic overuse, friction against the tubercle can result in tenosynovitis . Nondisplaced or minimally displaced distal radial fractures can produce tears of the extensor pollicis longus (EPL) tendon, typically distal to the extensor retinaculum [56,57] where the Fig. 10. Tenosynovitis of the extensor pollicis longus, extensor carpi ulnaris brevis, and extensor carpi ulnaris longus tendons associated with postfracture deformity of the distal radius. ** pollicis refers to the thumb.
Clinical presentation. May present as a transient locking of the thumb in flexion, followed by a painful snapping sensation during extension. Radiographic features Ultrasound / MRI De Quervain tenosynovitis was first described in 1895 as a stenosing tenosynovitis of the abductor pollicis longus (APL) and the extensor pollicis brevis (EPB) as the tendons course through the first dorsal compartment of the wrist.34,63 The first dorsal compartment is a tunnel that houses the APL and the EPB; however, in a percentage of the population these two tendons occupy their own space separated by a fibrous septum within the first dorsal compartment. Tenosynovitis and tendinosis of the abductor pollicis longus (APL) and extensor pollicis brevis (EPB) tendons is called de Quervain’s tenosynovitis. First described by Swiss surgeon Fritz de Quervain in 1895, 1 this condition is commonly encountered as an overuse injury.

De Quervain tenosynovitis is classically defined as a stenosing tenosynovitis of the synovial sheath of tendons of abductor pollicis longus and extensor pollicis brevis in the first compartment of the wrist due to repetitive use. (Redirected from Extensor pollicis longus) In human anatomy, the extensor pollicis longus muscle (EPL) is a skeletal muscle located dorsally on the forearm. It is much larger than the extensor pollicis brevis, the origin of which it partly covers and acts to stretch the thumb together with this muscle.
